Check for $75.00 – The
family limit is $200.00. The fee for Charter/Homes School/Private is $112.50.
*Please make sure all forms are signed and completed. FCHS
has a Registrar in the front office. Please make checks payable to FCHS. The Athlete will receive their Permit to
Practice Card – “white card” when these forms have been personally submitted to
the Athletic Office.
