Foothills Gateway, Community Centered Board
Community Centered Boards (CCBs) are private, nonprofit organizations designed to deliver services to individuals with developmental disabilities. Foothills Gateway, Inc. is a CCB that exists to empower and support citizens with cognitive disabilities, their families and other populations, as appropriate, to achieve their maximum potential and independence.
Services: include Resource Coordination/Case Management, Early Childhood Connections, Family Support Services Program (FSSP), Supported Living Services (SLS), Residential Services & HUD Program Administration, supported Employment Services, Work Services, Community Participation Services, Special Habilitation Services, Transportation and Adult Care Services.
Eligibility: persons with a developmental disability are those who have a "disability that is manifested before the person reaches twenty-two years of age, which constitutes a substantial disability to the affected individual, and is attributable to mental retardation or related to conditions which include cerebral palsy, epilepsy, autism, or other neurological conditions when such conditions result in impairment of general intellectual functioning or adaptive behavior similar to that of a person with mental retardation" (CRS 27-10.5-102). For children under five years of age, eligibility is based on determination of either a developmental delay or factors putting the child at risk of having a developmental disability.
Application: various services have extensive wait lists. It is best to call and request an assessment as early as possible.
