School to Work Alliance Program (SWAP)

School to Work Alliance Program (SWAP) is a collaborative between the Colorado Department of Education, the Division of Vocational Rehabilitation and community based organizations to provide counseling and guidance, job development, job placement, on-the-job training and job-site support to assist young people with disabilities to become employed and self sufficient.

Services: case management, career exploration, career development, employment related instruction, job development and placement and follow-up.

Eligibility: the individual must want to work, have the capabilities to work independently, be between the ages of 16 and 25 and must have already exited or will be exiting high school.

Application: the appropriate time for a SWAP referral is when an individual's primary focus is employment and they are available for services. However, as funding fluctuates and wait lists are enlisted it does not hurt to call early and inquire about the best timing for application. Referrals can be made by the home school through the Integrated Services department or counselors, by the Department of Vocational Rehabilitation Case Manager or directly by an interested individual.
